Iatrogenic bile duct injuries after surgery remain a substantial problem in gastrointestinal surgery. A variety of injuries can occur. Besides minor bile leakage of aberrant ducts, cystic stump or the main bile duct, complete occlusion of the main duct or a branch (often an aberrant right duct) can occur. When there is a biliary leak it is important to immediately drain the collection and understand where the leak comes from by direct opacification (percutaneously or endoscopically). Complex biliary leaks are associated with consequent strictures. The ESGE Guideline for the treatment of benign CBD strictures recommend multiple sessions of ERCP with temporary placement of an increasing number of plastic stents.
